Glazon Care Services Comprehensive Care License is also designated with the HCBS. Our approach is focused on providing care to our seniors in homes owned by us. Our homes are ADA (American Disability Act) compliant and are tailored to specific communities and cultures. The Benefits and Challenges of HCBS as outline by Medicare are as follows.
Cost effectiveness: Usually less than half the cost of residential care
Culturally responsive: Spiritual & Cultural activities support are available
Familiarity: Patient enjoys the comfort of a small community
Counseling or Clergy to assist with Bereavement
Accepts most wavier programs - EW, CADI, DD etc
Access to providers
Availability of qualified caregivers
Lack of 24/7 Caregivers (for traditional HCBS) - not Glazon Care Services
Family member may have to also provide daily tasks - care
Residents do not always want family member to care for them

Glazon Care recognized the many benefits and challenges which comes with HCBS. As a solution to the many challenges, we have set out to partner with the Minnesota Department of Health (in being licensed) and other foundation across the State. We are on a mission to create Culture Specific Communities - below are our plan for our CSC.

Culture Specific Communities (CSC) - is centered around the idea of providing a world class care and housing for people of similar background and heritage. In our view, when it comes to providing care, specificity is more important than generality. Imagine your family living in a community where the care providers and nutritionist are trained and sensitive to your specific culture. Perhaps, speaks your language, cooks your native foods and even transport you to your community specific events. Below are some of the benefits of CSC.

01. Do you have a standard housing contract?
Yes, we will provide you a standard housing contract to be review and during admission. The contract stands alone separate from the comprehensive care services.
02. What will it cost me to live in your Housing?
The price varies by space and number of occupant. For example, for families with fixed budget, we recommend semi-private rooms or suites. We also have discounts for couples or relatives living in a private suite. The price also varies based on your city or county. Please note, all price will be provide during our initial meeting with you.
03. Why could the housing with services ask a tenant to move out?
It is our mission to provide a world class services for all our tenants. Our intentions are not to displace any tenants. However we also have to be mindful disruptive tenants. Our staffs are training on techniques to help our tenants live harmonious with others. When intervention are not effective, we will help tenants find residence elsewhere.
04. What are limitations on services a client can receive from us?
We are a comprehensive care service provider, at current we do not have any specific limitations. Our care services range from independent to total care. If we are unable to provide suitable staffing or trained personnel on a specific service, we will let you know.
05. Do we offer opportunities for religious or spiritual practice?
Yes, our trained staff will help off you the opportunity to practice your faith. Depending on your service package, we also offer opportunity to transport residents to their place of worship. Our only policy is that, resident, or family of residents are not allow to impose their personal faith, spirituality, believe onto other residents or staff.
06. What opportunities and policies exists for tenants/residents and families to make recommendations about the building and services?
We provide survey, and opportunities for our tenants and their families to provide us feedback and suggestion as to how to improve the quality of the services we are providing.
